The undersigned, owner of Unit ___________, submits this front door application in accordance with Article VII of the Declaration of Covenants, Conditions, and Restrictions. Included with this application is a drawing/photo showing the design of the door to be installed.

I understand that the following guidelines are used to ensure that door designs are harmonious throughout the complex:

1. Maximum of 8 panels
2. A window is optional, but colored or stained glass is not permitted

I agree to accept full responsibility for the installation of the door and any subsequent damage resulting from the installation and use of the door.

If I choose to stain the door, I agree to install a door made of stain-grade oak with Behr Plus 10 natural stain. I agree that the maintenance and upkeep of stained doors is the individual homeowner's responsibility. I understand that the Association will not paint over stained doors. I also understand that the Board of Directors has the right at its discretion to take action against an owner that is not, in the Board's opinion, properly maintaining a stained door, and I agree to comply with all directions from the Board to keep my stained door properly maintained.

If I choose to paint the door, I agree to paint the door one of the colors designated by the Association and understand that subsequent paint maintenance of the door is the responsibility of the Association and will be included in the regularly scheduled paint maintenance of the complex.

If I choose neither to stain nor to paint the door, I agree to install a door made of fiberglass or other material that does not require painting or staining. The color of my door will be ____________. (Note: The color of the door is subject to board approval.) I agree to keep my door properly maintained in a good and attractive condition. At each periodic paint cycle, I will notify the Association not to paint my door. I understand that the Board of Directors has the right at its discretion to take action against an owner that is not, in the Board's opinion, properly maintaining an unpainted door, and I agree to comply with all directions from the Board to keep my unpainted door properly cleaned and maintained.

I agree that the work will be done in accordance with all city codes and, if required, that I will obtain the necessary permits.
